What is Perineoplasty?

This elective gynecologic surgery addresses any issues with the vaginal opening (introitus). The term perineum refers to the area between the opening of the vagina and the anus. Perineoplasty in Cincinnati is the operation used to correct either a small vaginal opening or a large gaping opening. This surgery repairs the tissue and tightens the muscles of the vaginal opening to make the perineum more aesthetically pleasing.

Perineoplasty can also be performed in conjunction with the Labiaplasty procedure, which is called a modified Perineoplasty. A modified Perineoplasty will only address excess skin in the perineum, it will not tighten the muscle or address a gaping vaginal issue.

Dr. Brenner has refined the in-office Labiaplasty using the ProNox device (inhaled nitrous oxide) and microtumescent anesthesia.

What Causes the Enlarging of the Vaginal Opening?

The most common cause is childbirth. During vaginal delivery, tearing of the perineal tissue is extremely common. As the tissue heals, uneven scarring and disfiguration often occurs, often leading to looseness of the vaginal opening. Conversely, a woman may feel the vaginal opening is too small or causes pain with intercourse.

Usually these symptoms start after having a tear or an episiotomy in childbirth. Often the pain is caused by excess scar tissue and reproducible on exam by a gynecologist. Child birth stretches or tears the perineum and/or vulva tissues causing structural issues with this area of the body. Excessive weight can also over-stretch and damage the structures of the perineum, causing a separation of the perineal muscles.

What Can I Expect During a Recovery From a Perineoplasty?

Following the procedure, you will be given a cocktail of medications to control pain, improve comfort during bowel movements. Expect to spend the first three to four days following the procedure on bed rest. Most women require pain medication for three to four days. You can expect swelling in the area for up to two weeks. After this time, you may return to desk work, but must avoid anything in the vagina (intercourse, tampons), heavy lifting, and soaking in water (bathtubs, swimming pools) for six to eight weeks.

Absolutely, this procedure is often performed in combination with other types of cosmetic surgery such as Labiaplasty Minora, Labiaplasty Majora, Clitoral Hood Reduction, and Vaginoplasty. May also be combined with the O-Shot® or other gynecologic procedures, such as MyFlow® or MyControl®.

Yes! Many patients have traveled across the country to have Dr. Brenner perform their Perineoplasty surgery. There are actually very few fully trained, and experienced cosmetic gynecologists in the United States. Our team can work with you for your travel needs and stay in Cincinnati, Ohio for your Perineoplasty. Initial consultations may also be conducted by phone and patients can also send photos of the area that needs treatment

What Kind of Anesthesia is Used for Perineoplasty?

We have a multiple step process to insure the highest comfort when performing the Perineoplasty. The area will be numbed with potent topical numbing cream. You will also be given oral and injectable pain medications and an oral sedative. We also have ProNox which is inhaled nitrous oxide that is used during the lidocaine numbing injections. Most of our patients do not even remember the numbing injections because they are using ProNox. After the area is anesthetized with the lidocaine injections, patients typically do not feel any discomfort during the procedure.
